Launch – Care to Independence

Dingwall’s Launch team of Personal Advisors offer a mobile, community based service to young people transitioning from care to independent living, across the Auckland area. This service is youth-led and provides support and advice to young people aged 15 – 19 years who have a goal of independent living.
 
The Personal Advisors provide one-on-one support to each young person, assisting them to identify their strengths and any areas they may need to know more about to help them manage their lives. From this a personal plan is made up that outlines the goals  the young person needs to meet in order to manage their lives independently.

Personal Advisors support young people to achieve their goals, linking them in with other services and resources where required and ensuring they have access to all that is available to them. 

The primary referral pathway requires the CYF social worker to complete the "Request for Joint Assessment Form" on the left of this page and submit it to Simon Reger at the Regional CYF office. This will provide for an NGO Personal Advisor, and the CYF Social Worker to complete an assessment of the young persons circumstances - particularly in relation to their transition needs. This assessment will recommend their suitability for acceptance on the Launch programme, or the equivalent service offered by Youth Horizons Trust called Ka Awatea.
